Runtime notes

Uses the original XML mapping files from the legacy app.
`Paragraph` mode is now the default output mode.
Uses MuPDF WebAssembly in-browser instead of the old PowerShell preprocessing path.
Page 5 is the default starting point for the supplied sample PDF.
